We are so glad you chose to attend Ohio State Theatre in show of support for a local fundraising effort!

Please complete the following information.

• Please allow at least 72 hours (3 business days) for processing.  
• Unless noted, all seats are reserved.  
• Tickets subject to availability.
• Best available seating will be assigned.
• If your 1st choice is not available, we will automatically assign your 2nd choice.  If neither choice is available, you will be contacted by a ticket office attendant.
• You will recieve an email confirmation once your claim form has been submitted.
• All tickets will be held at WILL CALL for pick up at the performance.
